The undersigned being now in ill
health and unfit for duty would respectfully
apply for leave of absence for a few days

Regt N.Y.S.M. having applied for a certificate
on which to ground an application for leave of
absence, I do hereby certify that I have care-
fully examined this officer, & find as follows
I was called in to see him on the 5th inst and
found him suffering from Congestion of
the Brain, produced by over exertion and
exposure to the sun. I find that his health
has been affected since the 21st July or the
day of the battle in which I understand
he took an active part. He requires a short
season of perfect rest mentally & physically.
He is therefore in my opinion unfit for duty.
I further declare my belief that he will not
be able to resume his duties in a less period
than two weeks.
